New Patient Registration

The doctors welcome new patients who live within our practice area. 

You can register online using the link https://www.eastlondonregistergp.nhs.uk/start

or simply fill in the prepopulated form (Click Registering Online) and drop the form along with your documents at the surgery.

If you are registered as a new patient, you will be asked to complete your medical card (or other form supplied by us, if you do not have your medical card) to enable us to obtain your full medical records from your previous GP.

You will also be asked to complete a lifestyle questionnaire and have a health check with our practice nurse before having a consultation with your doctor.

You have the right to express a preference of doctor (e.g. you might prefer a male or female GP) when you join our list and, whilst the practice will endeavour to comply, this may not always be possible. If this were the case, an explanation would be offered.

Non-English Speakers

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Online 'Registration' with the Practice

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and print to save time in the surgery.  Please bring this along when you present your identification to the surgery. On arrival to the surgery you will be asked to sign your form to confirm that the details are correct. You will be given a copy of the practice policy and there is a requirement to bring along your documentation.  An appointment will be given for a health screening before registration is complete.

You will be asked to complete a questionnaire on arrival at the surgery.

All children under the age of 16 will be requested to bring along a copy of their birth certificate.

Note that by sending the form you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Please print off your registration form, fill it out and bring it in with you on your first visit to the practice.
