To protect our other staff please do not visit our surgery if you are have any COVID-19 symptoms as new continuous cough, high temperature, loss of smell and taste. If you need to contact surgery, please use e-Consult for advice from your doctor, self help information or have an administrative request, email to the surgery or call the surgery.

COVID-19 vaccination have begun and patients will be invited according to the priority lists. Please do not call the surgery for COVID-19 vaccination as you will be invited from the surgery. Please note COVID-19 vaccines needs to be taken in two doses. The second dose will need to be 21 days apart from the first dose.

Please use E-consult for advice from your doctor, self-help information or have an administrative request.

Please contact surgery if you would like to sign up for online services that enables you to book appointments online, request medications online. You can also use NHS app to enjoy the service.

Online 'Pre-Registration' with the Practice

PreRegistrationIf you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will need to bring a printed copy along with you where will be asked to sign your form to confirm that the details are correct.

Pre-registration Form

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and print to save time in the surgery.  Please bring this along when you present your identification to the surgery. On arrival to the surgery you will be asked to sign your form to confirm that the details are correct. You will be given a copy of the practice policy and there is a requirement to bring along your documentation.  An appointment will be given for a health screening before registration is complete.

You will be asked to complete a questionnaire on arrival at the surgery.

All children under the age of 16 will be requested to bring along a copy of their birth certificate.

Please print off your registration form, fill it out and bring it in with you on your first visit to the practice.
