PCN Extended services in the surgery

We would like to encourage our patients to utilise the extended services that we have in the surgery. Please call the surgery to book appointment with one of the professionals regarding the support you require.

- Clinical Pharmacists deal with your medication review, medicines optimisation and medicines related issues including clinical discharge letters. We have three clinical pharmacists working from Wednesday to Friday every week.

- Two first contact physiotherapists working in the surgery from Monday to Wednesday every week.

- One Mental Health nurse working on site on every Tuesday.

- One Social Prescriber who supports patients with social issues and refer to range of local non clinical services. They take a holistic approach to people's health and wellbeing and connect people to community groups and statutory services for practical and emotional support.

The eligible patients will be contacted by Minuteful Kidney and delivery of the testing kits arranged upon agreement. For more information, please click Minuteful Kidney.
