New Patient Registration
The doctors welcome new patients who live within our practice area.
If you are registered as a new patient, you will be asked to complete your medical card (or other form supplied by us, if you do not have your medical card) to enable us to obtain your full medical records from your previous GP.
You will also be asked to complete a lifestyle questionnaire and have a health check with our practice nurse before having a consultation with your doctor.
You have the right to express a preference of doctor (e.g. you might prefer a male or female GP) when you join our list and, whilst the practice will endeavour to comply, this may not always be possible. If this were the case, an explanation would be offered.
You are NOT a registered patient with the surgery until you have been offered a health check with our Health Care Assistant. This appointment will be given when you submit your registration form to our registration clerk with your accompanying documentation Monday to Friday. A copy of the practice policy with regard to registration required documents may be obtained from reception.
